Frailty Team
Ruth Bryant is an Occupational Therapist by profession and a trainee advanced clinical practitioner. She has previously worked within the Care Quality Commission. This vast experience makes her the ideal person to lead our Frailty Team. This programme of work includes:
- completing frailty reviews and creating bespoke individual care plans to maintain good health and maximise quality of life
- signposting and referring people to community and voluntary services to bring activities and interests to enrich lives
- co-ordinating with other health services to ensure patients are accessing the services needed
- co-ordinating provision of support aids
The key focus of this project is to support frail patients to receive the care they need and support them in living well at home. Ruth’s endless passion and expertise means this part of our community is in very good hands.
