Our paramedics provide a home visiting service for housebound patients with acute or potential deterioration in their health and offer some in clinic appointments at some of our surgeries.

They can see patients across a wide age range depending on the patient needs and carry out visits across the whole WEB area.  They work in conjunction with the patient’s own GP and closely with the WEB PCN Community Frailty Team.

As part of their role our paramedics will:

  • Conduct thorough assessments of patients, identifying any medical, social or psychological needs, and create care plans accordingly
  • Provide management plans in conjunction with the patient’s own GP or the WEB PCN Community Frailty Team
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support the delivery of anticipatory care plans and lead certain community services (e.g., monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
  • Provide support and education to patients and their families, helping them to manage their conditions at home and promoting self-care, promote self-management where appropriate, including signposting patients to other community or voluntary services
