Pharmacy Team
The correct selection, use and monitoring of medication is essential to getting the most from your prescriptions. As part of our fantastic clinical team in WEB PCN, we have three Pharmacists and three Pharmacy Technicians on hand to provide expert knowledge and advice.
Working within the East Devon Covid Vaccination Centre, our pharmacy team were instrumental in the successful delivery of more than 70,000 doses of Covid Vaccine to the Communities of East Devon.
Our clinical pharmacy team is led by Louise Greaves who has previously worked within the Devon CCG Medicines Optimisation Team in addition to Community Pharmacy. She has an excellent knowledge of all the Practice teams across WEB, through her previous roles, strong connections with the Devon ICB and Royal Devon and Exeter Trust Pharmacists. Louise co-ordinates the programs of work to ensure that patients within the WEB PCN receive the most appropriate medication and have their medications optimised for the best possible outcomes.
The pharmacy team work within all our WEB PCN practices providing support in all aspects of medication management as well as carrying out prescribing safety audits, patient medication reviews and implementing medicine optimisation schemes. The pharmacy team are the longest standing service for WEB PCN and provide invaluable support for our GPs in all aspects of medication support.
